CCS is home to Caledon’s only accessible transportation program, the Exchange Community Hub that includes food support and a range of activities, the Caledon Specialist Clinic, and personal support services for seniors. Volunteer
Position Overview:
Functioning within a collaborative team model, the PSW works closely with Assisted Living clients living in their own residences to deliver individualized, flexible daily living support, thus facilitating increased independence and improved social, physical and mental health and wellness for clients.
Summary of Responsibilities:
Reporting to the Manager, Staff & Scheduling, within the Health Services Division, guided by a clear set of protocols and operating as a member of a service delivery team, the PSW:
• Assists clients with scheduled and unscheduled ADLs and IADLs on a daily basis;
• Assists with preventative care and emotional support to promote client health and well-being through active listening, reassurance checks and activation events
• Demonstrates high level of patience and a respectful and positive customer service attitude with clients, caregivers/families and other service providers;
• Uses Gentle-Persuasion techniques to encourage clients with challenging/responsive behaviours to comply with care delivery, thus minimizing the occurrence of missed or declined services;
• Ensures program integrity and client safety by strictly complying with Assisted Living Policies and Protocols such as health care plans, work schedules and medication reminder protocols, client confidentiality requirements and maintaining related documentation;
• Strictly adheres to procedures, regulations and laws concerning the storage and handling of controlled substances; Demonstrates proficiency with completing documentation and report writing;
• Conducts effective, thorough ‘shift-change’ discussions with colleagues at the beginning and end of every shift to ensure continuity of care for clients and to support each other in doing a good job;
• Trains/orients new PSWs on their shift to help them get oriented to the job;
• Oversees, coaches placement students as required (i.e. PSW practicum students, Life for Youth participants) and/or volunteers and sets a positive example of acceptable workplace conduct;
• Demonstrates excellent communication skills (written, verbal and interpersonal);
• Participates in regular team meetings to facilitate open communication, identify program issues review cases and provide advice on policies and procedures;
• Develops and maintains effective working relationships with colleagues and other service providers;
• Acts in a manner consistent with CCS Vision, Mission and Values, thus building trust and confidence among clients and the community.

• Must be able to work the Evening Shift 3:30 PM – 11:30 PM, 15 hours/week, Caledon Village (every other weekend and the Friday/Monday on the alternate week)
• PSW certificate from a recognized college and/or educational institution registered with the National Association of Career Colleges (NACC);
• Passionate about helping others; Effective Communicator;
• GPA or U-First Training an asset;
• Committed to working with a team to build a great customer experience;
• Valid Standard First Aid / CPR Level C;
• N95 Mask Fit Testing;
• Original, Clean Criminal Records Check – Vulnerable Sector;
• Up to Date Immunizations (Proof of TB Test); Proof of COVID-19 full vaccination;
• Experience providing personal care to frail/chronically ill and/or vulnerable individuals;
• Understanding of the concept of PSW ‘Scope of Practice’ and how to apply it in the work setting;
• Knowledge, skills and experience working with frail/chronically ill and/or vulnerable individuals including some with cognitive impairment due to early dementia, mental health issues, etc.;
• Valid drivers’ license, use of a road worthy vehicle, Able and willing to travel within Caledon in all seasons and varying weather conditions; Must carry a minimum of $1000,000 vehicle liability insurance;
• High level of patience, compassion and respect for vulnerable clients and a positive customer-service attitude;
• Demonstrated ability to function well in a team oriented work environment;
• Strongly developed time and stress management skills;
• Proficiency with Microsoft Office, Excel and Outlook a must, experience working in GoldCare an asset;
• Awareness that CCS provides PSW services 24 hours per day, 7 days per week (including Statutory Holidays) and flexibility to work varying shifts as needed;
• Awareness of and in agreement that while the incumbent may be assigned a ‘primary’ work location, CCS reserves the right to redeploy staff to other work locations throughout the Caledon area based on service delivery needs that may change from time to time for various reasons including (but not limited to): demand for service, new funding agreements, operational requirements, etc.;
• Fluency in spoken and written English a must;
• Access to a personal device (smartphone or tablet) is required.
